Jeffery Volunteer Types
(ACT 153 Clearances Requirements)
In order for you to be a casual volunteer in your
child’s classroom and/or school, you must sign the PDE 6004 Arrest/Conviction Report and Certification Form,
in the presence of the principal or principal’s designee. This form will be
required on an annual basis.
A casual volunteer is a volunteer who comes to a school infrequently and works in the presence of a teacher/administrator when with students. A casual volunteer does not have direct contact with children.
Examples include: Individuals who volunteer to assist in the planning or conducting of classroom celebrations; guest speakers; and concert/performance ushers.
In order for you to go on the field trips this school year, you must have the following clearances and a tuberculosis examination. Payments for these services are your responsibility.
* ACT 114 - FBI Clearance - https://www.pa.cogentid.com/index_pde.htm ($28.75 fee)
Please submit your 16-digit ID number, beginning with the letters PAE after your prints are scanned.
Register with Cogent through the above site. Make sure you register for the Department of Education FBI clearance (1st right-side menu box). Click register online and complete the entire application, using a credit card for the $28.75 fee. Print the Registration ID (16-digit PAE number). Take your receipt with registration ID and a valid photo ID to the most convenient fingerprinting site to have your fingerprints scanned (call the site first to be sure they are still open and for their hours of operation). Once you have had your fingerprints scanned at the fingerprint site, then you can submit your registration ID (16-digit PAE Number) to the school.
All Fingerprinting sites are listed on the cogent website. The Allegheny Intermediate Unit has extended its fingerprinting services to Mon.-Fri. 9:00 am-3:30 pm. Fingerprinting services will not be available on AIU observed holidays. Phone: 412-394-5700. Also, the fingerprinting site closest to Shaler Area School District is the UPS Store #5726, 5513 Wm. Flynn Hwy #400, Gibsonia, PA 15044; www.theupsstore.com/5726.htm; Phone: 724-443-0381; hours for fingerprinting are Mon.-Fri. 11:00 am-5:00 pm only.
Field Trip Chaperones will be required to renew their Act 114 FBI Clearance every three years unless the individual has been a resident of the Commonwealth of Pennsylvania during the entirety of the previous ten-year period and submits an affidavit attesting that they have not perpetrated or been convicted of any offense that would preclude their employment by the School District under Act 34 (Criminal History), Act 114 (FBI Criminal Background), or Act 151 (Child Abuse) or Section 6344(C) of the Child Protective Services Law.

Scroll down and click on, “New Record Check ” for volunteers only, then complete the Personal Information Section and follow the instructions to the Certification Page. Once the Search Results Table appears, click on the Control Number and write down the Control Number and your User Name and Password for future reference and use. The Record Check Details page is opened and this Record Check Details page is only a receipt and not acceptable as a “clearance”. Click on the Certification Page to access your official Clearance. Please print 2 Certification Pages, 1 for you and one for your Personnel File (an oval PA State Police Seal will be water-marked in the background on the actual Certification Form that you submit).

A new online application process for obtaining your Child Abuse Clearance is now available, using the above website. The phone number for questions is 1-877-371-5422 and phone hours are M-W-F 8:30 am-11:00 am and 1:00 pm-4:00 pm.
Click the box, “Create a New Account”; follow the instructions to complete the form, select reason for as volunteer and enter Shaler Area School District for the agency. When you have submitted all the required information on the form, you will receive an email confirming receipt of your Child Abuse Application. You will then either receive a second email when your application is approved, and it can be either instantaneously or take up to 2 weeks. You will print this form which is your actual Child Abuse Clearance Certificate.
Field Trip Chaperones will be required to renew their Act 34 Criminal History Report and Act 151 (Child Abuse) Clearance Statement each school year.
* Tuberculosis Examination - Prior to performing services for the School District, chaperones are required to undergo a tuberculosis examination in accordance with regulations of the Pennsylvania Department of Health and to furnish such report to the School District. Tuberculosis examinations must be less than 90 days old when presented to the District. As long as the volunteer maintains continuous service within the District, this exam does not need to be repeated. However, any break in service would require a new tuberculosis test.

are excused from obtaining the Act 114 clearance. Chaperones who meet this
